Student Medication Administration

To help us safely care for our students, parents/guardians must complete a Medication Authorization Form before any medication can be administered at school.

All medications must be provided in the original, properly labeled container. For prescription medications, emergency medications, or significant allergies, additional instructions from the student’s healthcare provider may be required.

Please complete the form below and submit it to the school office along with the medication.
